Field Nurse

The Field Nurse is responsible for conducting home visits for clients managed by the agency, including patient assessments and the clinical supervision of home care workers. The person in this role will ensure that the documentation of all professional nursing visits complies with New York State Department of Health regulations and agency policies and procedures.

Sunnyside Home Care offers compassionate care to more than 1,500 home-bound New Yorkers. Our skilled home health aides provide crucial support to help individuals remain safely in their homes and communities. Our administrative staff serves as the link between clients, their caregivers, and the professional aides in the home ensuring seamless coverage.

Responsibilities

  • Conduct a comprehensive initial in-home assessment of the client and the client’s environment in accordance with agency policy to ensure the client’s needs can be safely met in the client’s place of residence.
  • Obtain initial physician orders and review and revise the physician’s orders every six months and as needed in accordance with Department of Health regulations and agency policies.
  • Participate in the coordination of client care from admission to discharge, including discharge planning.
  • Develop, initiate, and evaluate the effectiveness of the client’s Plan of Care.
  • Oversee the clinical management of client services for all cases managed by the agency.
  • Conduct on-going monitoring and oversight of the client’s home care needs, and provide appropriate interventions as needed.
  • Identify potential client health/ safety concerns and intervene appropriately with client and/or family and the client’s physician.
  • Conduct client reassessments every six months or more often as needed, based on the client’s condition and the nurse’s clinical judgment.
  • Provide client/family education and make appropriate referrals to community resources.
  • Provide skilled nursing visits and direct care to clients in their place of residence
  • Provide supervisory and orientation visits as needed for Home Health Aide (HHA) supervision and orientation
  • Provide on-going communication/updates of the client’s condition to other members of the health care team as required by the agency or other contractual requirements.
  • Proactively collaborate with other members of the health care team to address clinical, social and service issues to ensure the highest level of quality care.
  • Ensure the ongoing compliance of client records to Department of Health regulations and agency policies is consistently maintained.
  • Provide orientation, on the job training, evaluations, and supervision for all paraprofessional home care workers in accordance with agency policy.
  • Participate in the agency’s quality improvement initiatives, including chart reviews
  • Provide feedback from in-home experiences to use as learning opportunities for enhancing the Home Health Aide training and In-service programs’ quality.
  • Network with social, health care and community organizations to promote positive relationships and referral activities.
  • Provide interagency clinical consultation.
  • Participate in the investigation and resolution process of client incidents and complaints.
  • Ensure client confidentiality is maintained with unrestricted access to client files.
  • Maintain an open line of communication with immediate supervisor and keeps them informed of issues/concerns promptly.
  • Participate in the agency’s on-call staffing plan on a rotating basis.
  • Comply with all agency policies and procedures.
  • Participate in staff development/in-service training to ensure client care is provided in accordance with current professional practice standards.
  • Participate in agency and department meetings as requested or assigned.
  • Perform other duties as assigned.

Qualifications

  • NY State Licensed Registered Nurse, BSN, and community home care experience preferred.
  • Good verbal and written communication skills.
  • Demonstrated competence in professional nursing skills.
  • Able to represent the organization with competency and professionalism.
  • Detail oriented and the ability to work well with people.

Salary Range:  $82,000 to $87,000       
Classification: Exempt
Reports to: Clinical Manager
