The Intake Coordinator/Case Manager will be a part of the Care Connections team that screens all new potential clients for their eligibility to any one of our senior services programs at SCS. This will require knowledge of all available programs and resources in the agency to properly direct a potential client to the appropriate services. The Intake Coordinator/Case Manager will then do a complete screen and intake for NYC Aging Case Management program which includes services such as Home Delivered Meals, EISEP Home Care, and Case Management services. They will also carry a caseload of APS clients. The Intake Coordinator/Case Manager will work under the supervision of the Care Connections Manager. Good record keeping, computer knowledge and time management skills are essential.
Responsibilities
- Assist with screening of clients in need of services.
- Conduct intakes and make referrals to other available resources.
- Identify and assist clients with basic information on benefits and entitlements.
- When necessary, contact emergency services when a client is in distress.
- Conduct and record periodic communication with APS workers to determine the status of clients.
- Input all required data in multiple data systems as required by NYC Aging and SCS.
- Participate in trainings, conferences, and agency meetings.
- Assist with outreach in the community.
Qualifications
- Experience in the human services field.
- Good verbal and written communication skills; must be bilingual in Spanish and English.
- Must have excellent interpersonal skills with an ability to establish rapport with staff, clients, informal supports, and other community and government agencies
- Must have a minimum of a bachelor’s degree.

Classification: Non-Exempt
Salary: $53,000 – $55,000
Location: We are currently working on a hybrid model: three days onsite, and two days remote.
