The Community Outreach Worker will build awareness of SCS’ Case Management services among local organizations, healthcare providers, social service agencies, and community groups. The right candidate will have strong communication skills, experience with relationship building, and organizational skills to effectively connect older adults with services and coordinate with community partners to promote program services.
When applicable for the job, SCS currently operates on a hybrid work model with three days in the office and two days remote.
Responsibilities
- Collaborate with the Manager of Communications & Public Outreach to promote the Case Management program and its services.
- Engage with the community to identify and refer eligible individuals to the Case Management Program.
- Deliver presentations to agencies, practitioners, and community groups in-person, via zoom or other social media.
- Serve as the main referral source for the Case Management Program.
- Be available for occasional evening or weekend outreach events, presentations, or community engagements.
- Perform other duties as assigned.
- Document outreach activities and referrals for program evaluation and reporting purposes.
Qualifications
- An associate’s degree in marketing from an accredited college/institution; a high school diploma with two or more years performing outreach activities.
- Knowledge of social services.
- Experience working with the aging population.
- Knowledge of communities within Districts 1,2,3, and 5.
- Bilingual (Spanish and English).
- Excellent communication skills (written and verbal).
- Engagement skills.
Physical Requirements and Working Environment
- Travel by all forms of public transportation or car in the community; ability to use stairs if there is no elevator available.
- Ability to communicate orally and in writing with outside agencies, clients, staff, co-workers and vendors.
- Organizational ability.
- Ability to track and maintain accurate data.
Classification: Non-Exempt Full Time/Temporary until 6/30/2026
Reports to: Case Management Director
Salary: $47,000 – $50,000 annually
