We are seeking experienced Registered Nurses to serve as Per Diem On-Call Nurses, providing clinical consultation during evenings, weekends, and holidays.
The On-Call Nurse works closely with the Service Delivery Coordinators, who handle incoming calls and operational matters. When concerns involve client health, safety, or clinical questions, the issue is escalated to the On-Call Nurse for review and guidance.
This role focuses on clinical decision support and escalation management. The On-Call Nurse does not conduct home visits during on-call hours but may recommend follow-up nursing visits during regular business hours and communicate those needs to the Clinical Manager.
Sunnyside Home Care is a Licensed Home Care Service Agency (LHCSA) that provides compassionate care to over 1,500 home-bound individuals throughout New York. We support clients in remaining safe and comfortable in their homes through coordinated care and dedicated staff.
Our nurses provide clinical oversight and guidance to ensure services are delivered safely and in accordance with each client’s Plan of Care.
Responsibilities
Clinical Consultation
- Provide clinical guidance when Service Delivery Coordinators escalate concerns involving client health, safety, or changes in condition.
- Assess reported situations and advise on appropriate next steps, including physician notification, emergency services, or follow-up care.
- Provide clinical support to address questions related to the client’s Plan of Care.
Collaboration with Service Delivery Team
- Respond to escalated concerns from Service Delivery Coordinators regarding client or Home Health Aide (HHA) issues that require nursing judgment.
- Provide guidance to support safe care delivery and resolution of clinical concerns.
- Communicate significant issues to the Clinical Manager or Director of Clinical Services as appropriate.
Incident Response
- Provide clinical guidance regarding reported incidents such as falls, changes in condition, or urgent client concerns.
- Ensure incidents are properly documented and communicated according to agency protocols.
Documentation & Communication
- Document all on-call communications and actions taken in accordance with agency policy and Department of Health requirements.
- Communicate any recommended follow-up visits or unresolved concerns to the Clinical Manager or clinical team for action during regular business hours.
- Maintain confidentiality in accordance with HIPAA and agency standards.
Other Duties
- Participate in orientation or training related to on-call procedures.
- Perform other related duties as assigned.
Qualifications
Licensing & Experience
- Current New York State Registered Nurse (RN) license required
- Minimum 3 years of nursing experience, preferably in home care, community health, or case management
- Current CPR certification required
Skills & Competencies
- Strong clinical judgment and problem-solving skills
- Excellent communication and documentation skills
- Ability to make sound decisions in time-sensitive situations
- Ability to work independently while collaborating with service delivery staff
Compensation: PM & Weekend On-Call Coverage: $50–$125 per day. Telephonic after-hours coverage for a Licensed Home Care Services Agency (LHCSA) from 5 pm – 8 am, Monday – Friday, and 24 hours on weekends to address urgent clinical matters.
