Social Work Supervisor – Caregiver Support Western Queens

The Supervisor provides direct supervision to five Care Managers (CM), using social work expertise to guide and support staff in delivering high-quality client care management services and monitoring service efficacy. The Supervisor oversees the appropriateness of client referrals, care plans, documentation, and overall service delivery to ensure continuity of care and prevent gaps in services to clients. The Supervisor is also responsible for fostering staff development, promoting accountability, and ensuring Care Managers meet programmatic expectations, productivity metrics, compliance standards, and quality outcomes through regular supervision, coaching, and performance monitoring in a supportive and collaborative manner.

The Sunnyside Community Services Caregiver Support Western Queens Program provides information, referrals, counseling and respite to family caregivers – those family members or friends who are providing care to physically disabled or cognitively impaired older adults, grandparents/kinship caregivers raising children, and older adults caring for a disabled adult. Respite service provides temporary relief from caregiving responsibilities on a limited basis and can include in-home care or adult day care services.

Responsibilities

  • Provide weekly individual and group supervision to Care Managers (CMs).
  • Guide and support staff interventions in complex, high-risk, or crisis situations.
  • Conduct regular case record reviews to ensure timely client contacts, accurate documentation, and compliance with program standards.
  • Monitor client services, staff productivity, and program metrics to ensure service goals and deliverables are met.
  • Coordinate trainings and workshops for staff on topics related to caregiving, including health issues, self-care, long-term planning, end-of-life planning, and related topics.
  • Provide direct client support and coverage in the absence of CM’s, as needed.
  • Collaborate with internal departments and external community partners to strengthen client supports, referrals, and service outcomes.
  • Work closely with the Director on program reporting, data tracking, quality improvement initiatives, and contract deliverables.

Qualifications

  • Master’s degree in Social Work (MSW) or related field required, with a minimum of two years of supervisory experience in social services.
  • Experience working in aging services.
  • MSW required. LMSW’s who are SIFI certified preferred.
  • Bilingual Spanish/English preferred.
  • Strong leadership, staff supervision, and team-building skills with the ability to work effectively with diverse stakeholders.
  • Strong organizational skills with the ability to manage multiple priorities, deadlines, and program responsibilities.
  • Experience with documentation review, data tracking, reporting, and quality assurance.
  • Proficiency in Microsoft Office, particularly Excel, and familiarity with electronic client databases; Salesforce experience a plus.
  • Ability to work independently and collaboratively in both office and community-based settings.
  • Excellent interpersonal, written, verbal, and public speaking communication skills.
  • Ability to engage effectively with clients, staff, and community partners from diverse backgrounds and cultures.
  • Other responsibilities as required.

Physical Requirements and Working Environment

Office work will be at our location in Long Island City, with flex space available at our main Sunnyside, 39th Street site, and telecommuting when applicable. SCS currently operates on a hybrid model with three days in the office, and two days remote. This is subject to change.

Start Date: July 1, 2026

Salary: $65,000 – $70,000 annually
