Through a partnership with PHI, SCS participated in a pilot program where four of our experienced Home Health Aides served as Care Integration Senior Aides (CISAs). In this role, CISAs support fellow home care workers and collaborate with care teams to help improve care for clients.
Working alongside our clinical and service delivery teams, CISAs provided additional support to home care workers in clients’ homes and served as a vital link among aides, clients, and families, and the care team. The program focused on identifying changes in clients’ needs, strengthening communication and care coordination, reducing preventable hospitalizations, and supporting the retention and professional growth of home care workers.
The initiative demonstrates the important role experienced homecare workers can play in improving both client outcomes and the overall quality of home care.
